Community Health Snapshots

 

Community Health Snapshots are a key communication tool for health advocates, program managers, city planners, and community groups. These one-page snapshots summarize health outcomes assuming past trends will continue through the year 2040. Understanding the past and looking toward the future is an important tool to strategically anticipate, prepare, and plan for healthier communities.

 

 

Health forecasting is a "valuable tool for predicting future health events and situations
such as demands for health services and healthcare needs. It facilitates preventive medicine and health care intervention strategies,
by pre-informing health service providers to take appropriate mitigating actions to minimize risks and manage demand."
